| What are these forms? |
| The online .DOT files are document templates that allow you to create an electronic form that you can fill out on your computer using the Microsoft Word97/2000/XP program. This has obvious advantages over completing the form by hand or with a typewriter. |
| What software do I need to use the forms? |
|
In order to
use the online .DOT templates, you need to have Microsoft Windows
9x/NT/2000/ME/XP and the Microsoft Word 97/2000/XP program. Don't have Microsoft Word? Click here to download a free MS Word viewer. Some documents are also available in Adobe Acrobat format (.PDF) These can be imported into the free Adobe Acrobat reader and printed. If you do not have Adobe Acrobat, please click on the icon below to download this free software:

| Will the form work with older versions of MS Word? |
| No. The form templates have been designed specifically for Word97 and up, and are not backwards compatible with previous versions of Word. Note: Converting the file to a previous version of Word or to another program such as WordPerfect, WordPro, etc. will change its formatting. |
| How do I install the file(s)? |
| After downloading the .DOT file to your hard drive, simply copy the file into the directory on your computer which holds your Microsoft Office document templates (this will be in the folder which contains your Office program). |
| How do I open or use the file(s) in MS Word? |
| With your MS Word97/2000/XP program open, select “New...” from the “File” menu. Click the appropriate tab for the folder where you copied the templates (see diagram below). Highlight the template and click OK. |
 Note: the above picture illustrates MS Word 97. Future versions of Word may differ slightly. |
| How do I fill out the form? |
| Fill in the form by entering information in each form field. You can use the TAB and SHIFT+TAB or the arrow keys to move between fields. |
| What do I do with the form once it's completed? |
| Once you have entered all the information required to be completed on the certificate, you can print the form just as you would any Word document. Please note that the form must first be signed by an authorized individual of your company, so do not fax the form directly from your computer. |
| Note:
Several of our dot files contain macros and as such will not properly
display and calculate data if the macros are disabled. Although we
have taken precautions to ensure all files are virus free, all files
downloaded should be virus checked locally. |
